First Things First! [Client Basic Information]
We need some basic information about you.
Open this form and fill it out. It's easy! When you have filled it in, FAX it to us at 732-560-9405.
This form has only ONE box to fill in! ok...TWO! But just fill in the SIX boxes and you'll be all set.
We'll need this filled out before we can start work, so go ahead and get it over with.
It's painless.
It is is similar to the form you would fill out when you see a doctor for the first time. (Except, we don't care what itches or how badly inflamed "it" is...)

It doesn't say we don't bite, because we do. But only every third person thru the door. So... you may get lucky :)

A couple of notes:
We only offer open accounts to commercial vehicles/fleets. Personal vehicles are not eligible for an open account. Owners of personal vehicles must pay in full when they pick up the vehicle. For convenience, we accept many methods of payment.
We do not offer "immediate approval" for completed credit applications. So, please be aware that all accounts are COD until our accountants have completed their credit investigation process. If you have NOT received a "credit approved" letter, your account is still COD and therefore, subject to the same requirements for payment upon time of completion & pickup.
